Administrative Fees

PAC charges for the following administrative tasks. Fees are subject to change and are not covered by insurance.

• Medical Records per chart – $25
• Returned Check – $40
• Special request physician letter – $15
• Completion of FMLA paperwork – $20
• Completion of additional school/camp forms – $10 (1st set free at Wellness Visit upon request)
• Missed Appointment Fee – $20
